Application process

Call the office or visit the website for more information. Requests can be made by a parent, legal guardian, the wish child, or a medical professional. The Make-A-Wish staff will contact the child's doctor to confirm that the child has a life-threatening medical condition and meets the medical criteria for a wish. Once a signed medical authorization form from the doctor is received, a volunteer wish team is assigned. The wish team then arranges a visit to the family.

Eligibility

- Because it is best if a child can fully comprehend their personal wish, a child must be over the age of 2 1/2 and under the age of 18 at the time of the referral. - The child has been diagnosed with a critical medical condition (a progressive, degenerative, or malignant medical condition that has placed the child's life in jeopardy). - The child has not received a wish from another wish-granting organization. - The family must be aware that the referral is being made. - Most qualifying conditions fall under one of the following categories: Cardiology, Endocrinology, Gastroenterology, Hematology, Immunology, Nephrology, Neurology, Oncology, Pulmonology, and Rheumatology.
