Requires individual or member of the family with a qualifying physical or developmental disability. Includes congenital developmental disabilities (formerly MR), traumatic brain injury, downs syndrome, cerebral palsy, spinal cord injury, paraplegia, muscular dystrophy, multiple sclerosis, and others.

Website 24 hours a day, 7 days a week. Applications accepted between October 15th and November 15th each year.
