Eligibility

- Must be a resident of Ohio. - Financial assistance is generally determined by a sliding scale of total household income based on the Federal Poverty Guideline. - Individuals eligible for financial assistance under hospital's HFA policy with an income level at or below 200% Federal Poverty Guideline low may receive free care. - Individuals with an income level from 201% to 400% of the Federal Poverty Guideline may receive discounted care based on a sliding scale.
