Offers a range of personalized supports for adults with intellectual and developmental disabilities and deliver them wherever they are needed - at home, at work and in the community. - Independent Living Supports: Provided by trained staff and tailored to the needs and goals of each person. Works with the individual to continue to master daily living skills and regularly participate in community activities and vocational programs. - Family Supports: Provides assistance to individuals who live at home with their families and wish to retain a sense of independence and/or ease the responsibilities of family caregivers. Provided by trained staff and tailored to the needs and goals of each person. Works with the individual to continue to master daily living skills and regularly participate in community activities and vocational programs. - Relief Supports: Offers respite care on an hourly or overnight basis. Provided as emergency support or when family members of the individual plan to be away for an extended period of time. Offers homemaker and personal-care services as part of this service including assistance with housekeeping, shopping, personal care, scheduling and transportation.
