- Serves low- and moderate-income residents of Avon and Avon Lake residing in ZIP codes 44011 and 44012. - Household must be at or below 175% the Federal Poverty Guideline Level. - Must have income. - For utility assistance: Must have shut-off notice. - For rent assistance: Must have eviction notice. - For mortgage assistance: Must be at least 90 days late with payment.
Will be requested as needed and include: - Photo ID. - Proof of address. - Proof of income. - Copy of bill(s) requesting assistance with.

Staff will take contact information from the caller and will pass the information on to the Saint Vincent De Paul Client Coordinator, who will contact the caller for screening, and make an appointment, if appropriate.
