Provides centralized access to services and opportunities for individuals with developmental disabilities age three and older. Services include but are not limited to: socialization opportunities, early childhood development, limited financial assistance for disability related needs, workforce development, assistance obtaining a driver's license, and housing opportunities.

Eligibility

<p>Open to residents of Brown County ages 3-5 who is one of the following:</p> <ul> <li>exhibits delays in at least two areas such as cognitive, sensory, communication, or social development</li> <li>has a diagnosed medical disorder with high probability to develop a developmental disability</li> <li>has a diagnosed developmental disability</li> </ul> <p>Also open to residents of Brown County ages 6 and older who:</p> <ul> <li>have been diagnosed with a developmental disability by a qualified professional</li> <li>exhibit delays in at least three areas, such as learning, mobility, self-care, etc. as determined through an assessment tool administered by Brown County DD</li> </ul> <p>Eligibility will be assessed and determined by intake specialist.
