- Serves residents in Wadsworth City, Wadsworth Township, and parts of Guilford Township, and Montville Township as well as open enrolled students.
- Children must be age five (5) by the first day of the school year in order to attend Kindergarten during the academic year.

- Child's immunization records.
- Proof of address/residency.
- Proof of custody.
