Eligibility

To qualify for HCAP: - Must be a resident of Ohio. - Must not be currently enrolled in Medicaid. - Must be an individual or family at or below 100% of Federal Poverty Income Guidelines or be a current recipient of the Disability Assistance program. - Services not covered by HCAP are medically unnecessary treatments, transplant services, physician charges, ambulance and patient convenience items (telephone, parking, television, personal items).
