Ages 0-21 whom have multi-system needs and are struggling to stay safe, stay together, and maintain everyday life and functioning within their family. It is a voluntary, service coordination service.

Referral to program. Families can refer themselves directly by calling (937)733-1490 or referrals can come from schools, juvenile court, mental health, or any other service agency/entity.

It is a free, and voluntary, service to any Preble County family that needs assistance navigating multiple agencies and resources to meet the needs of their child.
