Application process
- Fill out an application online at www.thedanefoundation.org. - After your request form is received, an email and/or text will be sent with the date and time for pick up of the Dane Relief Bag(s). - If the assigned time does not work, please email or text back to us to make other arrangements. - Items may or may not include, per availability: toothbrush, toothpaste, shampoo, soap, toilet paper, Purell/sanitizer, cleaning products or wipes. NOTE: One bag per person. Visit the website for more information. A Service Request Intake Form must be completed and can be downloaded from the website. Submit the Service Request Intake Form for consideration by email or mail.
