Income level at or below 145% of the federal poverty level (FPL). If the child is disabled, then the income must be at or below 150% of the FPL. On going eligibility may be maintained if the family's gross monthly income is at or below 300% of the federal poverty level. Caregiver must live in Lucas County and must be one of the following: Employed; Enrolled in an undergraduate program (associate or bachelor's degree); or receiving education necessary to maintain employment, certification, or licensure.
1) Proof of a qualifying activity for all parent caretakers in the household (such as work or school/training schedules)
2) Provide the name and address of an eligible childcare provider for each child in need of care (to find one, please use the search portal at: https://childcaresearch.ohio.gov/)
3) Proof of household income
4) Proof of any child support paid
5) Proof of citizenship or qualified alien status for children in need of care.
