Application process
- The application process may be started online through the Ohio Benefits self-service portal, over the phone or in person. Arrangements will then be made with agency. - Participants must re-apply once every two (2) months.
Fee
- No fee for those who qualify. - Some services may have small copays from $1 to $3.
Eligibility
Healthy Start or State Children's Health Insurance Program (SCHIP) is available to: - Uninsured children age 19 or under in families with income up to 206% of the Federal Poverty Guideline Level. - Insured children age 19 and under in families with income up to 156% of the Federal Poverty Guideline Level. - Pregnant women in families with income up to 200% of the Federal Poverty Guideline Level. - Residents of Jefferson County. - Legal resident aliens may have additional requirements to qualify. Must meet the following additional basic requirements: - Be a U.S. citizen or meet Medicaid citizenship requirements. - Have or get a Social Security number. - Must notify the county if there is a change in family circumstances that may impact eligibility. - Participants must notify the County JFS if there is a change in family circumstances that may impact eligibility.
