Description

Systematically coordinated, patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians in either a primary care or mental health setting. Services include lab, pharmacy, immunizations, infectious disease team, and reproductive health services (Family Planning of Northeast Ohio).

COVID-19 Temporary Message: Signature Health is now providing COVID-19 testing in Ashtabula. Clients who see Signature Health for primary care should contact their doctor. Someone who is not currently a patient of Signature Health should contact their COVID-19 testing hotline at 440-578-8203 Monday-Friday from 9AM-5PM to speak with a triage nurse.

Signature Health is continuing to offer all services. There are doctors, counselors, case managers, and supervisors on site every day. Hours of operation are 8 am to 5 pm Monday through Friday. Telehealth will be used as much as possible. Anyone who enters the building will have their temperature taken. If someone brings a patient/client they will be asked to wait outside.

  • New patients for Medication Assisted Treatment induction

Pharmacy hours of operation are 8 am to 5 pm Monday through Friday.
Prescriptions can be mailed or delivered (preferred methods) or picked up at the office.
