Application process

Complaints can be submitted online or by mail. Check website to look up businesses and charities, submit complaints, access business profiles, and other information through the website. Phone M-F: 9:00am-12:00noon and 1:00pm-4:00pm for information, to make a virtual appointment, or to speak to a representative. No walk-in service. Recorded information also available 24 hours a day, 7 days a week.

Free for complaints. Fixed fee for accreditation services.

Eligibility

Serves consumers in Clark, Darke, Greene, Miami, Montgomery, Preble, Shelby and northern Warren counties. For accreditation, business must have been in service for at least 6 months and meet and maintain specific accreditation standards.
