Eligibility
Serves honorably discharged Veterans residing in Carroll County who hold an Armed Forces discharge record known as a DD Form 2104 or DD214.
Required documents
A valid DD214 (DD Form 2014) displaying an honorable discharge status and two (2) of the following forms of identification (one (1) of which must contain a photo):
  • A Social Security card.
  • An original or certified birth certificate.
  • An identification card issued by the United States Department of Veterans Affairs.
  • A United States military identification card.
  • A license or permit to carry a concealed weapon issued by any state.
  • A motor vehicle operator's license that bears a photograph of the individual identified issued by any state.
  • An identification card issued that bears a photograph of the individual identified issues by any state.
  • A valid Passport that bears a photograph of the individual to whom the passport was issued.
  • A United States armed forces discharge record.
For Renewal:
  • Do not need to present military discharge papers.
  • Must present current Carroll County Veteran identification card or a proper form of identification displaying a photograph.
