Application process

Application required. Applications may be submitted online or by mail. Annual recertification required. Any address or eligibility changes must be reported within 30 days to continue service

Eligible customers must verify their eligibility through the National Verifier database. Customers may mail/submit their completed application and all supporting documentation to the Lifeline Support Center or they may go to an authorized Altafiber retail store where an associate may help them determine eligibility. No fee to apply. Call for information on fee waivers and monthly discounts. 

Eligibility

Household income that is at or below 135% of the Federal Poverty Guidelines, or participate in certain federal assistance programs, such as:

  • Supplemental Nutrition Assistance Program (SNAP)
  • Medicaid
  • Federal Public Housing Assistance (Section 8)
  • Supplemental Security Income
  • Veterans and Survivors Pension Benefit

Note: Lifeline benefits are limited to one per household per service.
