Also follows up with a Personalized Resource Plan to make sure the families are able to access the resources they need.

Most current or imminent kinship caregivers and adoptive parents over the age of 18 caring for children under 18 (or 19 if still in school, 21 if disabled) in the state of Ohio are eligible. Both formal and informal kinship caregivers are eligible.
