Application process

Contact to schedule a facility tour. Enrollment forms will be provided during tour. Prior to enrollment being finalized, the participant and their support team (parent/guardian, caregiver, SSA, etc.) must attend an intake meeting with the program director. Completed enrollment forms may be submitted at the intake meeting or sent to Client Services.

Fee

Vary by service. Financial assistance available for eligible participants.

Open to adults who have mild to moderate intellectual, developmental, or physical disabilities.
