If already connected with a hemophilia treatment center, center will assist. If not, call or e-mail at any time to request an application and be referred to a treatment center.

Eligibility

Must be an Ohio resident, age 21 or older, and have been diagnosed with a bleeding disorder. The family must be financially eligible based on the service needs of the patient and the adjusted gross income of the family, determined on a case-by-case basis.
