Eligibility

Income eligible pregnant women and families with children ages 6 weeks-3 years old. Must have verification of at least 35 hours of qualifying parental activities (employment, job training or school) for center-based services.

Required documents

Verification of income for the last 12 months or verification of TANF benefits or confirmation of Foster Parent status. Copy of child's birth certificate. Copy of child's medical insurance card. Verification of at least 35 hours of qualifying parental activities (employment, job training or school). Pay stub.

Mon-Fri 7 AM - 6 PM

Languages

Spanish, Somali, American Sign Language and any other language translation services as needed, upon request.
