Application process

Services begin with a comprehensive assessment. Call or visit online site to schedule an assessment appointment.

Fees

Accepts Medicaid insurances (Caresource, United Health Care, Paramount, Buckeye, Molina), Medical Mutual, Aetna, Anthem, and United Health Care. Also accepts and will directly bill to all private health insurances. Clients are responsible for remaining co-pays and deductibles. Family Pride offers a sliding fee scale for those clients who qualify.
