Provides guidance, medical care, personal support and information. In addition to coordinating care with your doctor, we’ll work closely with you and your family – mindful that you’re in control of your care. Hospice Services include: - Nursing care - Personal care - Medical care - Equipment and supplies - Medications - Therapy services - 24/7 availability for questions, support and home visits, as needed - Family and community grief support programs - Respite care - Emotional and spiritual support. If hospice is requested, an OhioHealth at Home hospice provider will visit your home and explain how we can best help you and your family. We also coordinate with your physicians to develop the best care plan for your needs.
