-Identification, -Insurance information, -Social security number of patient, -Current medication list, and -Custody or guardianship documentation (if applicable). If you are uninsured, -Proof of income, and -Proof of residency (can be a piece of government-issued mail such as a cell phone bill, utility bill, bank statement, etc.).

Monday - Thursday: 8:00 am to 6:00 pm. Friday: 8:00 am to 5:00 pm. Walk in hours: Monday - Friday: 8:00 am to 2:00 pm.

In person Spanish interpretation is available. All other languages are available via Language Line. Sign Language interpretation is available.

Walk in assessments are available. Specific appointments can be made by calling the location.

Sliding scale available; Medicaid/Medicare, and most private insurance accepted.
