Will need list of medications when making appointment. Health insurance card, if applicable. Those planning to request financial assistance services will need to provide proof of family income.

Fixed fee. Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis. For patients with insurance: program does not cover out of pocket expenses such as co-pays, co-insurance, and/or deductibles.
