Eligibility
  • Medical Eligibility:
Age 0–25 years Permanent resident of Ohio Under the care of a CMH-approved doctor (MD or DO) . Have or may have a chronic or special healthcare condition, such as cystic fibrosis, spina bifida, diabetes, heart defects, seizure disorders, cancer, hemophilia, or hearing loss.
  • Financial Eligibility (for the Treatment Program):
Determined case by case based on family income, medical expenses, and ability to pay Children receiving Medicaid or SSI are automatically eligible Certain income sources and personal assets are not counted, such as child support, stepparent income, or savings.

The Complex Medical Help (CMH) Program, formerly known as Children with Medical Handicaps (BCMH), is administered by the Ohio Department of Health. It assists individuals under the age of 26 who have special healthcare needs and are residents of Ohio. The program links families to a network of CMH-approved providers and helps cover costs for medically necessary services, including physician visits, therapies, surgeries, medications, and durable medical equipment. Ohio.gov +2 CMH operates through three main components:
  • Diagnostic Program: Helps identify or confirm special healthcare needs and develop a care plan. Services may include tests, x-rays, and evaluations by CMH-approved providers. Children are eligible until age 21, and financial eligibility is not required.
  • Treatment Program: Provides ongoing care for eligible medical conditions, requiring both medical and financial eligibility. Services include therapies, hospitalizations, medications, and service coordination. Children and young adults are eligible until age 25.
  • Service Coordination Program: Assists families in locating and coordinating services for children with specific diagnoses, often through hospital specialty teams. This program ensures families can access appropriate care and resources.
