GUARDIANSHIP SERVICES | LUTHERAN METROPOLITAN MINISTRY
Provides guardianship services for adults who are unable to make decisions for themselves and are deemed incompetent by the court. Guardians act as an advocate and substitute decision maker for the appointed individual and should be considered only as a last resort. Offers consultation, education, and information for persons and professionals considering guardianship as an option.

Application process
A referral must be made from social workers or other professionals from social service agencies, hospitals, attorneys, Adult Protective Services, or Probate Court. Before referral can be made, a Statement of Expert Evaluation confirming a substantial mental impairment must be obtained within a 90 day period prior to referral. Statement of expert evaluation and referrals forms are available on the Web site. Obtain and complete forms and return them by mail or fax. May call for information or for forms M-F: 8:30am-5:00pm. After referral is made, the individual is then assessed for possible acceptance into the program.
Fee
Fixed one-time fee if referred from hospital; fees vary in other situations, free in some cases.
Eligibility
Residents of Cuyahoga County who are adults 18 and over and lack available or appropriate family and/or support systems. Must have a substantial mental disability as defined by law (generally caused by mental illness or other conditions such as Alzheimer's, dementia, brain injuries). Does not take persons with developmental disabilities. There must be a compelling issue concerning the person's welfare that requires legal authority to act, such as health care, financial, or placement decisions. If not private pay, must have Medicaid or apply or have applied for Medicaid and the application is pending.
