Offers central intake for expectant women and families to provide resources and coordinate assistance by community health workers (CHW) in the client's preferred setting (home, agency, and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid. Completes a minimum of three (3) outreach attempts on each person screened to ensure all needs/opportunities have been met. Available resources may include:
  • Counseling services.
  • Peer Support.
  • Diapers and baby items.
  • Doula resources.
  • Food and clothing.
  • Job connections.
  • Safe sleep practices.
  • Safety support.
