Open to individuals age 18 and older who:

have been a resident of Boone, Campbell, or Kenton County for at least 60 days
have no insurance or limited insurance that covers dental services
have a household income at or below the limits listed online

Application required. Applications may be completed online. Other required documentation may be dropped off in person at the County Health Center or the NKY Health District Office.
