Ages 18 and over with a diabetes diagnosis.
Health insurance card, if applicable. Those planning to request financial assistance services will need to provide proof of family income. A written referral is often required from a physician in order for services to be covered by insurance.

Fixed fee. Third party payment accepted, including Medicaid, and Medicare. Individuals who are uninsured or underinsured, and who have family incomes at or below 200% of the yearly Federal Poverty Guidelines (FPG) are eligible for free medically necessary services. Those with family incomes between 200-400% of the yearly Federal Poverty Guidelines (FPG), who have no insurance, or whose out-of-pocket responsibility is substantial after insurance benefits are exhausted may need pre-approval to receive services.
