Eligibility

Open to residents of Warren County age 60 and older who require assistance with certain activities of daily living. Serves individuals who are not eligible for services through another payer, such as Medicaid, Medicare, insurance, or hospice. Eligibility for specific services is determined by a care manager during an in-home assessment.
