City of Columbus families with children age 12 and under.

Proof of Columbus residency (i.e. utility bill if address is not on picture ID). Proof of parent or guardianship. Photo ID (driver's license, green card, passport or state picture ID) for parents (ID from other countries is also accepted). Child(ren): Original birth certificates, passport or custodial papers with child's birth date. Child's ID must have the date of birth on it to be valid. Care Source cards without date of birth will not be accepted as proof of age.

All applications will be cross-checked with other Firefighters 4 Kids agencies' applications. Duplicate applications will be discarded, and this may result in not being able to receive holiday gift assistance.
