Non-emergency Medical Transportation For Medicaid Recipients | Hamilton County Job And Family Services

Provides non-emergency transportation (NET) for Medicaid recipients. Individuals can receive transportation to and from medical providers. Transportaion authorization may be in the form of bus passes, van services, Access tickets, or mileage reimbursement. Transporation requests must be submitted at least five days before transportation is needed. Individuals must be able to transfer to and from a wheelchair independently.

Application process

<p>Appointment required. Individuals may call 513-946-1000, ext 6 or complete and return a <a href="https://www.hcjfs.org/services/resource_assistance/medical_assistance/non-emergency_transportation/transportation_request.php" target="_blank" rel="noopener" class="underline text-blue-700">transportation request form</a>. To schedule an appointment, you must have both customer and medical provider information. Customer information includes name, address, phone number, social sercurity number, and date/time of medical appointment. Medical provider information includes doctor's name, name of doctor's building or hospital, address, and phone number.</p>

Fee

Transportation is paid through Medicaid.

Eligibility

Open to residents of Hamilton County who are both eligible for and currently enrolled in Medicaid.
