Application process
Appointment required. Individuals may call 513-946-1000, ext 6 or complete and return a transportation request form. To schedule an appointment, you must have both customer and medical provider information. Customer information includes name, address, phone number, social sercurity number, and date/time of medical appointment. Medical provider information includes doctor's name, name of doctor's building or hospital, address, and phone number.

Transportation is paid through Medicaid.

Description
Provides non-emergency transportation for Medicaid recipients. Individuals can receive transportation to and from medical providers. Transportation authorization may be in the form of bus passes, van services, Access tickets, or mileage reimbursement. Transpiration requests must be submitted at least five days before transportation is needed. Individuals must be able to transfer to and from a wheelchair independently.
