Provides 2 barrier-free homes for 16 adults (8 adults at each location) with intellectual and/or physical disabilities who require 24 hour support services. Home includes nursing staff on site. These services include assistance with personal care, meals, community integration, transportation and all aspects of daily living.

Referral must be made by the Cuyahoga County Board of Developmental Disabilities M-F: 8:30am-4:00pm.

Eligibility
Age 18 or older. The agency targets those with developmental disabilities (such as epilepsy and cerebral palsy), traumatic brain injuries, muscular dystrophy, spina bifida, and multiple sclerosis, spinal cord injuries. Must be Medicaid eligible, willing to participate in UCP's Day Program, and have the ability to benefit from the program. Persons requiring 24-hour nursing care or who have behavior management requiring restraints cannot be accommodated.
