Provides skilled nursing and home health care including professional nursing assessment, medical services such as blood pressure checks, medication monitoring, blood sugar checks, wound care, catheter care, and tube feeding, behavioral health care (limited to dementia and depression), home health aide services, homemaker and companion services, temporary illness support programs, and rehabilitation services including physical, occupational, and speech therapy. Short-term in home respite care is provided as well.
