Eligibility

Serves residents of Ohio. A physician, physician assistant, advance practice nurse, optometrist, or chiropractor must provide a written prescription to verify applicant's disability.

Required documents

BMV Form 4826 (Application for Disability Placards) and original written prescription. The prescription must have the following information: - Name of the person with the disability. - Indicate patient is applying for a disability placard or similar wording. - The health care provider must date and sign the prescription. - How long the disability is expected to last. The health care provider must specify an ending date or indicate that the disability is permanent. Placards expire on the date specified by the health care provider.

Fees

The fee for a new, replacement, or temporary placard is $5. A permanent placard that does not expire costs $15. Accepts the following types of payment: cash, check, money order, and debit/credit card. When paying by debit/credit card, there is an additional transaction fee.
