Community Clinics For Youth | Lorain County Health & Dentistry
Provides primary and preventive health care for youth. Services include physical examinations, evaluation of growth, development, nutrition, vision, hearing and emotional health, health education, school and sports pysicals, immunizations/vaccines, assistance completing applications and referrals to specialists. NOTE: Assistance with transportation to appointment may be available.

Fee
- Accepts Medicaid/Medicaid Managed Care, many commercial insurance plans, and private pay. - Minimum fee of $20 for medical clients who qualify for a 100% discount based on Federal Poverty Guidelines. - Sliding scale available for youths age 17 years and under that have a letter from Job and Family Services denying health care coverage. - Call the office and speak with the Billing Department for details.
Eligibility
Serves youth ages birth to 19 residing in Lorain County and surrounding communities.

Spanish speaking staff member and utilities language interpretation services.
