Provides outreach services in the home for seniors. Screenings such as blood pressure, cholesterol, hearing and vision, and appointments with a podiatrist are periodically available. Helps seniors complete various forms such as Medicaid forms. Offers a bereavement support group and a Parkinson's disease support group.

Application process

Phone for information or to make an appointment M-F: 8:00am-4:30pm. Many services need pre-registration in person. Must arrange for transportation in advance.

Fee

Free; Fixed fee for podiatry and some screenings, but may be paid through Medicare.

Eligibility

Males and females 60 and older. Outreach available to Solon residents. Other services targeted to Solon residents, but may be available to non-residents.
