Eligibility
  • Serves residents of Medina County with a household income at or below 200% of the Federal Poverty Guideline Level.
  • Client can receive service once every 30 days.
Required documents
  • Current proof of address/residency (such as utility bill).
  • Photo ID.
  • On initial visit, client will complete a form to self-declare income.
