Outpatient Care Coordination | University Hospitals Parma Medical Center
A registered nurse and social worker provide assistance to patients/families, including home evaluation, patient assessment, and recommendations for future care. May include, but is not limited to, help with nursing home placement, or education of the patient in current residence which might help prevent unnecessary ER visits or hospital stays. Help is also given with required financial and health care forms.
