Assists with the Medicaid process for individuals diagnosed with a severe mental illness, or have been diagnosed with one of the following chronic conditions: such as Cancer, HIV/AIDS or Immune Deficiencies, End Stage Renal Disease, Sickle Cell Anemia, Cystic Fibrosis, Hemophilia. Serves those who have had a previous transplant as well as those who are active on the solid organ or soft tissue transplant waiting list.

Application process

- Clients may fill out initial application online at benefits.ohio.gov or call the Shared Services Line. - Individuals who are not already on Medicaid must be referred by their health provider. Those already on Medicaid should contact their caseworkers.

Fee

- No fee to enroll. - There may be small co-payments for some services.

Eligibility

- Must live in Harrison County and be a United States (U.S.) citizen. - Serves individuals with a severe mental illness, or who have been diagnosed with chronic conditions such as certain malignancies, HIV/AIDS or Immune Deficiencies, End Stage Renal Disease, Sickle Cell Anemia, Cystic Fibrosis, Hemophilia, and those who have had a previous transplant; in addition to those who are active on the solid organ or soft tissue transplant waiting list. - Those with chronic conditions must also be over 21, receive Social Security disability benefits, have one or more inpatient admissions due to the chronic condition, receive a qualifying score on the Adult Needs and Strengths assessment, and reside in an approved Home and Community Based Services setting. - Individuals who are not already on Medicaid must be referred by their health provider. - Those already on Medicaid should contact their caseworkers. - Financial eligibility is an income up to 225% of the Federal Poverty Level.
