Fee
Free assessment. Some individuals may be eligible to receive services for free, but some clients are required to pay a monthly co-pay .The package of services must not exceed a certain percentage of the costs of Medicaid nursing home care.
Eligibility
Ages 60 or older and needing assistance with personal activities (like bathing, dressing, preparing meals, etc.); Medicaid-waiver eligible (PASSPORT has higher income and asset eligibility requirements compared to traditional Medicaid - call for more information). Must live in Adams, Brown, Gallia, Highland, Jackson, Lawrence, Pike, Ross, Scioto, or Vinton counties.
