Provides educational programming to help eligible participants manage chronic diseases such as asthma, arthritis, heart disease, and type two diabetes. Goal is to slow the progression of degenerative illnesses and conditions as long as possible without the use of more intensive health care services. Teaches relaxation techniques, healthy eating, managing sleep and fatigue, managing medications and monitoring blood sugar, exercise, and communicating effectively with healthcare providers.
