Mycare Ohio | Ohio Department Of Medicaid
Provides information and counseling regarding MyCare Ohio, a managed care program for Ohioans who receive both Medicaid and Medicare benefits. MyCare Ohio includes all of the services that are covered in the following home and community-based Medicaid waivers: PASSPORT, Choices, Assisted Living, Ohio Home Care, and Transitions Carve-out.

Hours
Cases will be reviewed every 12 months. Individuals with household changes that might affect eligibility in between review times should let their caseworker know within 10 days. Ohio Medicaid Consumer Hotline hours: Monday - Friday: 7:00am to 8:00pm. Saturday: 8:00am to 5:00pm. Closed on Federal holidays.

- Call the Ohio Medicaid Consumer Hotline during regular hours of operation or visit the website at www.ohiomh.com/resources/mycareohiofaq for more information. - Search online at www.ohiomh.com/home/findaprovider?program=mycare to find out which MyCare Ohio plan a doctor, hospital, clinic, or medical professional accepts. - MyCare Ohio members may enroll for both Medicare and Medicaid services, to maximize coordination of care and benefits. These members are called “Dual Benefits Members”. MyCare Ohio members may also choose to opt out of signing up with a managed care plan provider by enrolling as “Medicaid Only Members” solely for Medicaid benefits, while maintaining traditional Medicare or a Part C (Medicare Advantage) plan that is not a contracted MyCare Ohio managed care plan. - Managed care plans available in Lorain and Medina Counties include Buckeye, CareSource, and United Healthcare. - Managed care plans available in Summit and Portage Counties include CareSource and United Healthcare.
Fee
No fee to enroll in the program with a managed care plan provider.
Eligibility
Ohioans MUST enroll in a MyCare Ohio plan if the following apply: - Age 18 and over. - Live in one of 29 MyCare Ohio counties such as Medina, Lorain, Portage, and Summit counties. - Receive both Medicaid and Medicare.
