Eligibility

Children and youth ages five years or older who may have extensive histories of abuse, neglect, other forms of victimization, or behavioral health needs and who are experiencing foster care or are at risk of being removed from their home and primary family.

Description

CB-ESC enhances youth and community safety through individualized care plans created collaboratively with youth, families, and service providers. We also promote placement stability by offering comprehensive support to caregivers, helping them more effectively care for youth with significant needs. Our program reduces placement disruptions and improves youth safety and well-being through intensive support and timely access to essential services including: - 24/7 in-home crisis intervention and support services - Case management, including community resource linkage - Parent support services, including behavior management, trauma-responsive guidance, and conflict resolution - Educational advocacy and support - Comprehensive behavioral health services.
