Health Clinic provides medical and nursing services for individuals who have asthma, COPD, or other lung health needs. Services include: - Physical exams - Health assessment - Asthma education - Chronic Obstructive Pulmonary Disease (COPD) education - Nutrition, exercise and weight management counseling - Chest x-rays, EKG’s, breathing tests - Influenza and pneumonia immunizations - Social services - Community outreach, and referral to other social service agencies - Medical supplies, nebulizers, peak flow meters - Prescription assistance programs - Assistance with medical supplies and pharmacy prescriptions -Wellness services - Diagnostic laboratory services - Preventative services - Home care follow-up for seniors and asthma care. - Sports and work physicals.

Fee
Clients must also be eligible for the HEAP program based on household income. The total household income of an applicant must be at or below 200 percent of the federal Fiscal Year 2012 federal poverty guidelines.
Eligibility
Must be within 200% of Poverty Level for the Lung Health Clinic and 200% of the poverty level for the Mobile Lung Health Clinic, as pre-screened by either the Breathing Association Home Energy Assistance or IMPACT Community Action Agency, and cannot afford to pay for health care, and have not seen a doctor in over one year.
