Provided on an individual fixed fee for service basis. The State can, in some cases, use Medicaid funds, which would normally be used to support the individual in a skilled nursing or long term care facility, to pay for this service instead and avoid premature institutionalization.

Provides a wide range of assistance to individuals with disabilities in their own homes or apartments. Services are individually tailored to each client's needs, and may include assistance with personal care, financial management, shopping, transportation, and more depending on each individual's needs. Service is carried out on the basis of an annual contract, and consumer decides whether or not to renew each year.
