Waiver Services | New Avenues To Independence
Provides a wide range of assistance to individuals with disabilities in their own homes or apartments. Services are individually tailored to each client's needs, and may include assistance with personal care, financial management, shopping, transportation, and more depending on each indvidual's needs. Service is carried out on the basis of an annual contract, and consumer decides whether or not to renew each year.

Application process
The County Board of Developmental Disabilities and the individual together identify a need for services. Then the Board contacts licensed providers who, if they feel they can meet the need, will submit a proposal. The individual and family will meet with the providers who apply, the individual will choose a provider, and then a contract will be drawn up for services. The agency can be contacted M-F: 8:30am-5:00pm. Most referrals for Supported Living and Community Based services are received from Northeast Ohio County Boards of Developmental Disabilities. However, arrangements can also be made directly with New Avenues for private referrals.
Fee
Provided on an individual fixed fee for service basis. The State can, in some cases, use Medicaid funds, which would normally be used to support the individual in a skilled nursing or long term care facility, to pay for this service instead and avoid premature institutionalization.

Must have diagnosis of a developmental disability. Serves Cuyahoga, Ashtabula and Lake Counties.
