Eligibility

Must be in the last stages of a terminal illness and wish to have a treatment of pain and symptoms rather than curative treatment. Service is provided to those residing in Cuyahoga, Lorain, Medina, Summit, Lake, Geauga, Portage, and Ashtabula counties.

Offers in-home hospice and palliative care for those with terminal and/or degenerative illnesses. Provides skilled nursing care and other services as needed. Offers supportive services, such as art therapy, massage therapy, social work services for patients, and counseling, spiritual and bereavement services for patients and their families.
