Eligibility

Serves those who need palliative care/pain management due to a degenerative disease or condition, but whose symptoms are not yet severe enough to warrant hospice care. Serves the following counties: Cuyahoga, Lorain, Medina, Summit, Lake, Geauga, Portage, and Ashtabula counties.

Description

Provides pain and symptom management for those with early to mid stages of degenerative or terminal illnesses. Once symptoms become more severe, patients will be referred to hospice.
