Complaints can be filed about a physician, osteopathic physician, podiatric physician, physician assistant, anesthesiologist assistant, massage therapist, cosmetic therapist, acupuncturist, mechanotherapist or naprapath practitioner by using either the online complaint form or downloadable complaint form.
