Application process

Phone for participating eye doctor or check the website once student is referred to a special education program. Appointment must be scheduled within 90 days of the initial diagnosis (unless student had an exam within the previous nine months). Phone local eye doctor for appointments. Uninsured need to request a voucher for payment that will be sent directly to the eye doctor of their choice from the participating list.

Fee

Fees vary by eye doctor and service. Organization will pay full amount for uninsured for eye exams and eye glasses.
